UPDATE: The fix was to log out on the Google Fi app and then log back in, clear Google Phone app data one more time, then go into it, turn off Visual Voicemail and turn it back on. Did you try deactivating visual voicemail and then enable it again? If that does not work you can clear the data of the phone app.
